Financial Disclosure Statement On-line Filing Instructions
If you do not already have an account, you will need to first register for one. Otherwise you may login using the user name and password you created upon registration. If you have forgotten either your user name or password, please use the links provided below the login box to recover them.
When registering for a new account, you will need to provide a unique user name and password. The password must include a minimum of 8 characters, including at least a letter, a number, and a special character (such as “@”; “#”; “&”; “$”; “%”; “*”). You will also be required to provide a valid e-mail address.
An e-mail including an activation link will be sent to the e-mail address provided. You must first activate the account by clicking on the link in the e-mail.
Once inside the FDS system, you can then fill out the specific information that will be reflected on the final FDS statement.
You do not need to complete your statement in one session. Your answers will be retained through subsequent logins until your statement is filed.
Please answer each question and hit the button to add the information. The button is also available to delete any information once entered.
To access instructions related to each question simply click the instruction button at the top right of each page.
Additional information can be added in the form of a PDF document. A browse feature is available for attaching these documents.
Once you have completed all of the questions, you can preview the disclosure statement before submitting it to the Ethics Commission.
You can print or e-mail a copy of the previewed form to a designated liaison before submitting it to the Ethics Commission.
Once the FDS filing is complete, you can electronically sign and submit the form to the Ethics Commission. At this point, if you later determine that any of the information provided is incomplete or incorrect, you will need to follow the Commission’s addenda process. Once filed, a unique confirmation number is assigned to the statement and the ability to save the statement as a PDF file is provided to the filer. An e-mail confirmation is also sent to the filer.
If you are required to pay a filing fee, an on-line payment is accepted via credit card or electronic check (ACH). There is no additional cost to you for using a credit card or ACH payment. An e-mail receipt will also be provided to you after the on-line payment is verified.
